Job summary

The post forms part of a community based multi-disciplinary resource team which aims to promote the independence of patients/clients living at home/within the care sector so as to avoid inappropriate hospital admissions, and facilitate and ensure safe early hospital discharge to prevent/reduce any unnecessary long-term permanent placements.

Main duties of the job

The Cardiff and Vale UHB Community Resource Teams are looking to recruit a motivated and dynamic Occupational Therapist with experience of working with older adults in community or hospital settings.

The post is based in the Vale Community Resource Service at Barry Hospitalin Barry. The Community Resource Teams are made up of health, social services and third sector staff all dedicated to working in patients' best interests to support independence at home, avoid inappropriate hospital admissions and ensure safe and early hospital discharge.

The team provides a seven day service with a weekend rota in place.

The successful candidate will be an experienced practitioner who is able to apply their knowledge of Occupational Therapy and acquired clinical skills that are underpinned by research, evidence based practice and continued professional development.We can offer:-Expert professional support and supervision, peer support, regular team meetings and in-service training-Continuing professional development opportunities-Involvement in service developments, including opportunities for you to contribute to audit, research and projects relevant to your clinical work-A friendly, enthusiastic and supportive multi-disciplinary working environment
