Lead Clinical Nurse Specialist in Myeloma

Job summary
The post holder will have responsibility for developing and supporting this speciality service to provide equitable care to patients presenting with Myeloma Cancer and act as a resource to carers, the Multi-disciplinary colleagues and the primary health care team. This will be achieved by the post holder ensuring a smooth transition of care from initial referral through diagnosis, treatment and beyond treatment into the survivorship stage, establishing links between the acute and primary care group, supporting both patient and carers to ensure a robust stratified follow up process for cancer patients.
The post holder would liaise with services within Wales and with other key partners such as Health Boards, Primary Care, Voluntary Services, Social Services and Hospices. The post holder will provide expert nursing advice and support to patients and their families at all stages of disease.
The main responsibilities of the role will be related to clinical expertise with an emphasis on initiating, performing and maintaining standards of clinical excellence.
A Non medical prescribing qualification would be desirable but can be taught.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The duties required for this role are:
- Develop the knowledge base of other nurses and health care professionals caring for patients with myeloma
- Initial patient assessment and initiating a care plan for patients in partnership with the wider multidisciplinary team (MDT).
- Supporting patients whilst they are undergoing diagnostic tests
- Assessing prescriptions to maximise patient management.
- Providing patients, their families and staff with expertise (in-depth knowledge and understanding of Myeloma)
- Undertaking medicine management for myeloma patients.
- Initiating and implementing standards of nursing treatment and care for myeloma patients
- Developing new approaches to running services
- Undertaking strategic planning to incorporate specific speciality needs and evidence-based practice
- Providing nurse-led clinics as appropriate such as pre and post transplant and inpatient/outpatient
- Undertaking HN
Other duties will include
- To request and organise chemotherapy treatments, stem cell collection and transplant work up.
- To liaise with community palliative care teams and other community based agencies to provide community support and specialist care for those that require it.
- To be actively involved in myeloma clinical trials at UHW and to be the main point of contact to facilitate recruitment of myeloma patients from across South Wales into appropriate trials in conjunction with the clinical trials team.
- Network locally and nationally to ensure best practice
