Job summary
The Emergency and Acute Medicine Directorate (EAMD) is looking to recruit Trainee Advanced Nurse Practitioners who are enthusiastic and innovative to support the exciting development of our Acute Medicine Same Day Emergency Care Model (MSDEC).
MSDEC is an acute assessment and diagnostic area for ambulatory patients that is supported with Acute Physicians and physicians associates. We are looking for trainee ANPs with a proactive and confident approach to managing patients referred via primary care or the emergency unit. Patients are triaged and assessed with urgent investigations and treatment being delivered as part of the same day emergency care approach or via the virtual ward that supports the function of MSDEC.
The Virtual Ward is an innovative approach to allow patients acute assessment and investigation without the need to remain in hospital. The service model will enable existing acute medicine patients to be discharged home from hospital earlier than would have traditionally been possible. It is an alternative pathway for those patients who do not require acute medical in-patient support and are clinically fit for discharge but do require on-going investigations and treatment planning.
To meet the four pillars of advanced clinical practice you will provide autonomous clinical decision-making within your scope of practice as well as supporting the ongoing transformation work using your leadership, teaching and audit skills.
Main duties of the job
Apply clinical patient assessment and diagnosis skills to the Acute Medicine patient group who attend the MSDEC. You will work within your developing scope of practice to autonomously discharge or refer to specialty teams as required. Working across all areas of the MSDEC.
Support and facilitate the development of clinical practice through research and education as part of the multi-disciplinary team.
Provide leadership within the clinical team and support ongoing service improvement and development through audit and quality improvement projects.
Contribute to the teaching and education programme of the Emergency and Acute Medicine Directorate for the multidisciplinary team.
Independent prescriber or working towards.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
