- To strategically develop and implement diabetes services across the Gibraltar Health Authority, in line with Public Health initiatives, working as a member of the diabetes team, to improve the quality of diabetes care.
- To assess, plan, implement and evaluate care for diabetic patients.
- To provide clinical supervision for other nurses and members of the MDT and specialist advice on diabetic care.
- To contribute to the education and training of other staff and patients.
Main duties of the job
- Support the triage of diabetes patient referrals to the diabetes team, assessing need and urgency of the referral, deciding when the patient needs to be seen and by whom.
- Educating others using an advanced expert knowledge base derived from Diabetic clinical experience, on-going reflection, clinical supervision, and engagement in continuous professional development.
- Monitor service provision, activity and training undertaken. Manage and implement change relating to the diabetes team, monitoring and evaluating the change to improve quality of service provision.
- Develop and disseminate, in collaboration with key stakeholders, policies, guidelines and care pathways for diabetes management to promote evidence-based practice and integrated services.
- Undertake regular quality control of blood glucose meter used in clinics and home visits.
- Maintain accurate records of Hba1c results.
- Maintain accurate records, submitting statistical returns and compiling reports as required.

- Synthesising and interpreting assessment information particularly history including prior treatment outcomes, physical findings, and diagnostic data to identify Diabetes and to determine at risk and subnormal states of health.
- Demonstrating timely use of diagnostic investigations to inform clinical decision making.
- Exhibiting comprehensive knowledge of therapeutic interventions including pharmacological and non-pharmacological advanced nursing interventions for patients with Diabetes.
- Manage and deliver nurse led and multi-disciplinary outpatient clinics for a complex caseload of people with diabetes, particularly insulin initiation and maintenance clinics.
- Support use of insulin pumps and glucose monitors as part of integrated and holistic care of Type 1 Diabetes.
