Job summary

Hillingdon is London's second largest borough by area. To support the health and wellbeing of our residents the council works in close partnership with the North West London Integrated Care Board (NWLICB) and the local borough-based partnership (BBP) known as Hillingdon Health and Care Partners (HHCP). HHCP comprises of Hillingdon Hospitals NHS Foundation Trust, Central and North West London NHS Foundation Trust (CNWL), H4All, a partnership of voluntary sector health care providers; and the body that brings together Hillingdons GPs known as The Confederation.

The joint ambition of the council and health and care partners is to improve the health and wellbeing outcomes of our population as well as to reduce the variation that exists in health outcomes for people across the borough by delivering more integrated care. Population health management (PHM) is at the centre of our approach to integrated care in Hillingdon and three new posts have been created to drive this forward. The posts will form part of a new PHM team that is being established.

Main duties of the job

You will be reporting to the PHM Programme Manager, working within a small team delivering a wide range of projects and activities that progress the planned PHM programmes/projects to reduce health inequalities. The post holder will be expected to work both autonomously and collaboratively to support health and care system-wide activities.

Reporting to the PHM Programme Manager to support delivery across all areas of the PHM programme, the key responsibilities of the post include:

  • Leading on the day-to-day planning and delivery, reporting, and monitoring of programmes/projects to ensure they are delivered to agreed timescales.
  • Working in partnership with managers , clinical staff and wider professionals across HHCP to ensure support and engagement for projects/schemes.
  • Ensuring all project/scheme risks and issues are identified, recorded, and managed effectively.
  • Developing key performance indicators and evaluation approaches in collaboration with service leads to demonstrate that sustainable change is delivered.
