ROLE PURPOSE:

The London Borough of Hillingdon Staying Close Project is an initiative that provides ongoing support for young people transitioning from the care system into adulthood. It aims to support care experienced young people to maintain connections with former caregivers and offer resources to help them succeed in independent living. The project focuses on stability, guidance, and a sense of belonging during this critical phase of their lives.

The Staying Close Transitions Nurse is responsible for supporting young people as they transition from the care system into adulthood. They facilitate and coordinate the transition process, assess, and address health needs, collaborate with multi-disciplinary teams, empower young people, provide education and training, and contribute to service development. Their role is to ensure that young people receive comprehensive support, guidance, and advocacy during this critical phase of their lives.

The development and delivery of a comprehensive, high-quality transition service for care experienced young people.

To promote and ensure that these young people are empowered to move into adult services.

Facilitate, support, and coordinate the transition of care for young people with health and/or complex health needs who are preparing to leave children's services or the care system. This includes face-to-face contacts with the young person, their family, foster carers, and coordination with the wider Transition Service and Multi-Disciplinary Teams.

Support the transition for care leavers based outside Hillingdon who are likely to return to Hillingdon for their adult life.

Provide clinical expertise on healthcare to young people, looked after children, care experienced young people, their families, carers, and multi-disciplinary/multi-agency teams. Deliver and empower the young person with relevant health promotion to meet their individual needs.

Take the lead in coordinating the transition process for care experienced young people with health and/or complex health needs or life-limiting conditions.

Ensure the production of a Health Summary/Health Passport, in agreement with the young person.

Ensure that safeguarding children and safeguarding vulnerable adult principles are adhered to at all times throughout the transition process for all care experienced young people and reflected in pathway and service development.

Enable care experienced young people with or without health and/or complex healthcare needs to make a smooth transition into adult care services by ensuring strategic planning of all their care needs.

Maintain knowledge of the current Team Plan and understanding of own contribution in order to ensure delivery of this plan.
