Main duties of the job
No two days are the same, youll be working with people who come into contact with us to have conversations to discuss what matters to them and others around them. Youll have a can do attitude, a good awareness of the local information and advice that is available and use this to support people and their carers to solutions, drawing on what they already have around them that is working, or could work. Youll work in partnership with others across social care, Health, Probation, and Housing teams.

independently with adults (and their informal carers) who have highly complex care
and support needs and/or where there are court of protection issues and
safeguarding adult concerns.
Make, and implement, judgements and
decisions with autonomy on cases with high levels of complexity, risk,
uncertainty and challenge with the oversight of the manager.
Plan and
implement interventions designed to address needs and reduce risk.
Prioritise tasks appropriately and be
responsible for own work.
Liaise
with partner agencies to secure support and services for people drawing on
support, their families and informal carers.
Lead on
S42 (safeguarding adult) investigations and Court of Protection processes.
Manage/Chair
a range of multi-agency meetings designed to monitor and progress Safeguarding
Adult Protection Plans and Vulnerable Adult Risk Management.
Maintain
up to date records and produce written reports.
Complete
legal documentation and file and serve legal papers where required.
Provide
expertise in Social Work Practice for less experienced workers by leading by
example when practising and by assisting others with complex work when help is
