Job summary

Hartlepool Borough Council are seeking to recruit an enthusiastic and motivated community nursery nurse to work in their 0-19 Early Help Service as an integral part of the wider multidisciplinary team; to provide an effective, high quality service to families.

Main duties of the job

As a community nursery nurse based in the Early Help Team you will be working with children predominantly aged 5 and above and their families to manage their growth and development and to build their skills, whilst also supporting them to manage their health needs in line with the Healthy Child Programme.

You will work in a variety of settings, including the family home and school settings. You will also offer support in both group settings and on an individual basis and in accordance with service need.

The post holder will contribute to the health needs assessment of children, young people and their families. Deliver a defined plan of care guided by the School Nurse / Health Visitor/ Locality Manager / Clinical Lead.

To promote the health and well-being of children, young people and their families, so they may be enabled to reach their full potential and not be disadvantaged through ill health or disability.

Be part of the team in developing, implementing and evaluating programmes including the Healthy Child Programme.

To work with the School Nursing, Health Visiting teams and wider multi agency team including other partner organisations, with responsibility for providing quality services which promote the health and wellbeing of children and young people.
