Job summary
The
post is a 30-hour Social Work post workingin the Hospital Social Work
Team based at York District Hospital. The team is well established and is staffed
with experienced Social Workers and Social Care Workers.
Main duties of the job
As
a social worker in the Hospital
Team your role will require you to work within a fast paced
environment, within a supportive Hospital Social Work Team facilitating timely
and safe hospital discharges. The team work to facilitate rapid hospital
discharge and also complete assessments post discharge in line with the
Discharge to Assess Guidance. You will hold your own caseload and will work alongside the Discharge Hub and
within the multi disciplinary teams based on the wards. The team cover York Hospital
and the community hospitals based in the
city and at Selby and Easingwold, so
regular travel between these locations is required as part of your role,
a pool car is available for these journeys. You will receive regular
supervision and be supported by the hospital social work management team; we
have monthly team meetings and peer support.

Qualifications
Essential
- A professional qualification in Social Work and be registered with Social Work England as a Social Worker
- Demonstrate and uphold Social Work England Professional Standards
- A sound understanding and application of legislative frameworks underpinning adult social care, for example the Care Act 2014, and the Mental Capacity Act (MCA) 2005, Mental Health Act 1989 and the Human Rights Act 1998
- An understanding of statutory social work, including critical awareness of current issues and new evidenceinformed practice research
- Demonstration of an ongoing commitment to continuing professional development and be able to evidence this in their everyday practice and supervision, sharing CPD opportunities and learning from training events
- Be able to identify and take up opportunities for professional development and training relevant to the role
- Recognition of the high level of confidentiality required in working with people, their carers and families and help to manage sufficient security of such information, recognising, being able to justify and take appropriate action when the right to privacy is superseded by professional or legal requirements
- Ability to work as part of a team and take a multi-agency approach where needed, working alongside other professionals to secure positive outcomes for people
- Demonstrable critical knowledge of a range of theories and models for social work intervention
