Job summary

An exciting opportunity has arisen for Senior Reablement Assistants to join the team at our new Intermediate Care Centre. The centre will provide short term rehabilitation (for up to six weeks) which enables people to regain as much independence as possible in skills for daily living (personal care, meal prep, medication, mobility, use of equipment etc.).

Main duties of the job

The successful candidate will: hold a level 3 Diploma in Health and Social Care or equivalent or commit to achieving this within 1 year of commencing employment; hold the HSC level 3 Medication Unit or have a commitment to achieve the unit; numeracy and literacy level two or equivalent; have extensive experience of working in a care/health setting; have an understanding of the ethos of Reablement.For further information contact Lisa Mussett on 07901 330045.

Job responsibilities

Job Purpose: To participate in the provision of a Reablement Service in the Intermediate Care Centre for a period of up to six weeks to help prevent unnecessary hospital and care home admissions and to facilitate Early Supported Discharge from hospital. To participate in the provision of a Reablement service to enable individuals to regain or maintain their ability to live independently as far as possible. To participate in the monitoring and management of complex health conditions through the measurement and supervised treatment of baseline vital signs to enable people to be cared for at home To undertake non-complex assessments for people with physical disabilities and arrange the provision of appropriate equipment in order to maximise independence To participate in the implementation of specialised rehabilitation techniques as part of a recovery programme Relationships: Accountable to: deputy manager Accountable for: N/A General Contacts: Occupational Therapists, Physiotherapists, Community Nurses, and Social Workers/Care Managers, People using the service, Carers, Care providers, GPs Key duties and responsibilities: 1. To participate in the planning, development, monitoring and reviewing of care plans and goals for individuals within the Intermediate care centre, in conjunction with relevant professionals within a risk management framework. This will include attending planning/ review meetings and case conferences. 2. To participate in the provision of reablement and /or rehabilitation programmes to assist people to learn or regain skills as part of an agreed therapeutic plan, which may be in conjunction with Occupational Therapists and Physiotherapists, and will involve team working and working on a one-to –one basis. 3. To be responsible for day to day decisions on varying a person’s programme. 4. To participate in the provision of a Reablement Service by providing emergency personal care within the centre to help prevent unnecessary hospital and care home admissions and facilitate an Early Supported Discharge service. This will include providing support with personal care, administering medication and monitoring vital signs. 5. To undertake baseline vital signs observations accurately recording and reporting all results appropriately to include; - temperature, pulse, blood pressure, blood glucose monitoring, urinalysis, weight and height under the supervision of Community Nurses. 6. To act as a mentor and supervisor as necessary to junior members of staff and participate in the delivery of the induction package to newly appointed staff. 7. To undertake assessments for people with physical disabilities and arrange the provision of appropriate equipment and minor adaptations in order to maximise independence under the supervision of Occupational Therapists 8. To implement specialised rehabilitation techniques as part of a treatment and recovery programme under the supervision of Occupational Therapists and Physiotherapists. 9. Following assessment to advise people of appropriate adaptive techniques within own skills and remit using own initiative and knowledge of positive risk management. 10. To work with members of the multi-disciplinary team to ensure high quality support for all individuals using the service, enabling optimum potential to be reached. 11. To provide emotional and practical support to individuals and their informal carers through the process of transition and change. 12. To maintain written records and reports in accordance with the service’s Policies, Procedures and Practice Guidance. 13. To assist in the management of medication, in accordance with the care plan and in line with the Medication policy and procedures. 14. To participate in the supervision and appraisal process and demonstrate an active commitment to continuous professional development 15. To attend relevant training courses identified as part of the supervision and appraisal process and to participate in team development programmes. 16. To comply with the services Policies, Procedures and Practice Guidance and ensure that professional standards are maintained at all times. 17.
