We are looking for passionate and driven individuals to join
our Talk Community Healthy Lifestyle Team. The service provides integrated
behaviour support for smoking cessation, weight management, alcohol reduction,
physical activity and falls prevention, all embedded in sustainable behaviour
change and positive wellbeing.
As part of a new exiting opportunity the post holder will
work within Wye Valley Hospital trust and Herefordshire and Worcestershire Health
& Care Trust to deliver an inpatient stop smoking treatment programme.
The post holder will
Work with hospital admissions teams to support the
identification of patients admitted to hospital who smoke
Offer a personalised tobacco treatment programme,
commencing during their inpatient stay and continuing post discharge with
access to nicotine replacement therapy and behavioural support
Work with the community element of the service and other
partner organisations to ensure the delivery of stop smoking support post
discharge
Work with staff teams across WVT to embed the long term
commitment of the inpatient smoking cessation treatment programme
Main duties of the job
The ideal candidates will be motivated, organised and
proactive advisors, with experience of working within a healthcare environment
and delivering holistic community health programmes, passionate about promoting
healthy lifestyles and delivering motivational change which is client centred.
The successful candidate will also be confident and
experienced in delivering support to groups and 1-2-1 meetings.
We are looking for individuals with strong communication
skills, who are confident delivering holistic health and lifestyle advice to
individuals and groups, including carrying out initial assessments and brief
interventions using proven behaviour change techniques, being able to adapt and
change their delivery techniques to suit vulnerable individuals to develop
increase and strengthen their skills to lead a healthier lifestyle.
A background in smoking cessation, nutrition, exercise,
public health, community health, recovery or support working and experience of
working within a health care environment would be beneficial along with being
excellent team player and having a willingness to learn and develop.

Working within the hospital setting this role will work directly with identified smokers on admission as an inpatient to deliver evidence based smoking cessation advice and support in line with the requirements of the NHS Long-Term Plan.
They will offer practical tools and techniques, advice, knowledge and encouragement to enable and motivate people to change smoking behaviours and achieve better health choices and goals for themselves, their families and the wider community.
Support the wider community participation and responsibility for improved health outcomes through working with the community element of the health trainer team.

To engage with individual smokers or groups of smokers identified on admission to hospital to provide specialist advice and support to stop smoking.
Identify, make contact and work with smokers during their inpatient stay
Deliver 1-1 or group sessions on smoking cessation to encourage behaviour change and health improvement.
Develop and maintain relationships with individuals who are experiencing the greatest inequalities in health;
promote the equality and value the diversity of individuals;
Build and maintain contacts with the hospital and community settings in line with the requirements of the NHS long term plan.
To communicate with individuals, their families and groups about health and health improvement in relation to smoking:
Provide information about the benefits to health and wellbeing of stopping smoking to staff and patients.
Provide innovative ways of improving health and wellbeing
Provide information about the relationship between behaviours and health;
Enable individuals and groups to develop their knowledge and skills about health and wellbeing;
Help individuals to access and use local services
Signpost individuals to other agencies for information, support and resources.
