Were looking for 2 experienced Occupational Therapists to join our supportive Long Term Occupational Therapy Team, helping adults with complex and long-term needs live safely and independently at home.
Youll deliver person-centred interventions, lead on Disabled Facilities Grant referrals, promote Assistive Technology and digital tools like AskSARA, and contribute to innovative projects across health and social care.

Main duties of the job
- Work as an integral member of a multidisciplinary team, delivering high-quality occupational therapy interventions within a community setting. Contribute your OT expertise to Integrated Care and Decision Making (ICDM) clusters alongside GPs, community matrons and a range of key healthcare professionals, supporting those with complex and long term health conditions.
- Provide tailored support to individuals with a range of needs, including physical and mental health conditions, learning disabilities and age-related challenges. Empower service users to develop or retain their independence through meaningful occupation. Work alongside Care Providers to promote client centred, proportionate care delivery for those with long term care needs.
- Take a lead role in the development of Disabled Facilities Grant (DFG) referrals and initial design briefs. Champion solutions that enable individuals to remain safely and independently in their homes and communities.
- Apply a sound working knowledge of relevant legislation and guidance, including the Care Act 2014, Housing Regeneration Act, Disabled Facilities Grant Guidance, Health & Safety legislation, and Building Regulations.
