Reablement Occupational Therapy Assistant

We’re looking for a dedicated Occupational Therapist to join our Reablement and Independence Service (RIS). This is your chance to make a genuine difference in people’s lives — helping residents regain confidence, build independence, and stay well in their own homes.

You’ll be part of a friendly, multi-disciplinary team working across the community, supporting people with a wide range of needs — from physical and mental health conditions to learning disabilities and age-related challenges. You’ll also work closely with colleagues in Local Access Points, Locality Teams, and Hospital and Discharge to Assess services to make sure support is joined-up and focused on what matters most to each person.

  • Pay Level:  5 £30,160 - £35,530 fully inclusive of Local Weighting Allowance
  • Contract Type: Permanent
  • Working Hours: Flexible Rota 7 days a week, including weekends, bank holidays and out of hours as required. Rotas are subject to change based on the service's needs.
  • Location:  Slough, Berkshire/Community care surrounding areas (people’s homes in the community and visiting hospitals or care home if required as part of the Reablement interventions)

In this community-based role, you will support residents in regaining or maintaining independence through strength-based assessments, rehabilitation, and recovery-focused interventions.

Working within a multi-disciplinary team, you will assist Occupational Therapists with residents who have a range of needs, from physical and mental health challenges to learning difficulties and age-related conditions. You will also collaborate with social care and discharge teams to ensure smooth care transitions.
