Lincolnshire County Council has an exciting opportunity in for aSocial Worker Level 1position based within the Adult Care Team at Pilgrim Hospital, Boston.This role would be suitable for a newly qualified Social Worker requiring their ASYE or a Level 1 candidate looking for more experience in a hospital setting.
About the Role
You will be undertaking assessments of need both within hospitals and the community settings to ensure our customers are able to liveindependent lives and for those that needsupport, the rightsupportpackage, in the right place, at the right time.
The customer is at the centre of the process; our highest continuing aim is to provide the best service possible to service users and their families.
The aim of the team is to work hand in hand with NHS and third sector staff to ensure that people we support are at the very centre of their own discharge. Working creatively when planning support packages, thinking outside of the box to ensure an individual care plan is in place that empowers the service user to participate.

You will work as part of the 7-day Adult Care rota. This equates to approximately 1 in 4 weekends worked, and these are all scheduled 3 months in advance.
