As CHC Coordinator, you will:
- Supportstaff in applying the NHS CHC Framework.
- Jointly manage complex cases with adult care practitioners.
- Ensure guidance, training, resources, and practice standards are in place.
- Monitor CHC activity using appropriate systems.
- Represent Lincolnshire Adult Care on panels,supportdispute resolution, and maintain effective relationships with NHS partners.
