In this dynamic role, you will work alongside the current CHC Coordinator, bringing your operational experience and expertise in NHS Continuing Healthcare to further strengthen the team.This is an ideal opportunity to balance direct practice with leadership and development responsibilities.

As CHC Coordinator, you will:

  • Supportstaff in applying the NHS CHC Framework.
  • Jointly manage complex cases with adult care practitioners.
  • Ensure guidance, training, resources, and practice standards are in place.
  • Monitor CHC activity using appropriate systems.
  • Represent Lincolnshire Adult Care on panels,supportdispute resolution, and maintain effective relationships with NHS partners.
