If you are an experienced and registered Nurse or Social Worker, our Louth & Horncastle Learning Disability Team would be excited to hear from you.

We are delighted to offer this recruitment opportunity for anexperienced Nurse or Social worker(known internally as Qualified Practitioners) within our Louth &Horncastle Team. This is a full time permanent position- 37 hours per week.

The Qualified Practitioner role is both an exciting and challenging role. For those that love fast paced and make a difference work, this is the position for you. The Learning Disability Team is proud of the Practitioners we have in our team. They take great pride in doing their job well and are passionate about providing the best service they can for the people who access our specialist service.

Main duties of the job

You will have an allocated case load which includes complex cases.Working in a Person-Centred way, carrying out strength based Adult Needs Assessments and Reviews focusing on what the person can do rather than what they cannot do. Working with the people wesupportand their families to maximise independence.

Our aim is tosupportpeople to reach their goals and aspirations. Whether that be employment, community access or moving into their own home. We help people to overcome the obstacles and challenges they may face,supportingthem to reach their potential.

You must have use of a vehicle, be able to travel effectively and within a reasonable timescale to meet service requirements, this could include beingresponsiveto last minute unplanned visits which may be non local/out of county appointments.

Our core office hours are 08:4517:15 (Mon-Thu), 08:4516:45 (Fri).We currently work a mixture of home based and office working, however there will be a requirement to be in the office when required to meet the needs of the service.

  • Knowledge of legislation relevant to provision of health and social care for people with learning disabilities including the requirements of the health and safety legislation and Valuing People.
  • Knowledge of health and health promotion issues specific to people with learning disability.
  • Knowledge of screening procedures and how they are used to effectively target resources.
  • Interpersonal, verbal and written communication skills
