Job summary
We are looking for innovative and motivated Live Well Navigators to work as part of Live Well, Health Improvement Services, Public Health, in support of individuals who live in Swindon who have one or more long term health conditions and would benefit from coaching and behaviour change interventions. The role of the Live Well Navigator is to work alongside the individual with a person- centred approach to improve their health, wellbeing and quality of life.
Within this role you will have the ability to build strong relationships with health, social care and the voluntary sector services in Swindon as well as finding ways to reach out to those facing greatest inequality and be creative and collaborative in your approach, inspiring and motivating lasting change.
You must be fluent in the English language - as a requirement of Part 7 of the Immigration Act for the effective performance of a customer-facing role.
You will be subject to an enhanced Disclosure and Barring Service Check and the ability to travel throughout the Borough will be required and access to a car

Main duties of the job
You will be delivering a 1:1 intervention and advocating for the people you work with at multi-disciplinary team meetings called TAP (Team Around the Person) meetings. You will work closely with the Project and Team Lead, Programme Manager along with colleagues in the wider team and partners across the health and social care system and will focus on ensuring that the Live Well offer is evidence based, person centred, well planned, safe and effective.
You will find ways to reach out to those facing greatest inequality and are creative and collaborative in your approach, inspiring and motivating lasting change.
