Job summary
We are looking for a motivated and well-established Live Well Navigator to work on a pilot project in partnership with Health, Adult Social Care and other agencies to provide support for people in Swindon who are at risk from escalating crisis. Your excellent knowledge of behaviour change and motivational interviewing make you the perfect candidate for this role.
Within this pilot, you will build rapport and through conversation, develop a detailed understanding of the persons perspective and ensure that relevant agencies are invited to participate in a Team Around the Person meeting. At this meeting you will support or advocate for the resident and ensure that an action plan is developed to reduce the impact of crisis. You will then ensure that the action plan is delivered and reviewed whilst providing the individual with the opportunity to overcome barriers, better self-manage their health and create new positive networks that improve resilience and self-efficacy. You will have strong partnership building skills to nurture relationships with colleagues across the voluntary and community sector to ensure that Swindons rich and vibrant community life is accessible to all.
Main duties of the job
You will be delivering a 1:1 intervention and advocating for the people you work with at multi-disciplinary team meetings called TAP (Team Around the Person) meetings. You will work closely with the Project and Team Lead, Programme Manager along with colleagues in the wider team and partners across the health and social care system and will focus on ensuring that the Live Well offer is evidence based, person centred, well planned, safe and effective.
You will find ways to reach out to those facing greatest inequality and are creative and collaborative in your approach, inspiring and motivating lasting change.
