Main duties of the job

Health Visitors are a valued part of our team, who work with the multiagency team to empower children, young people and families to make healthy lifestyle choices. They are able to identify when extra help may be required and offer the appropriate support or signposting as needed. Swindon

The successful candidate will have excellent communication skills and able to build effective working relationships with a wide range of people. They will have exceptional time management and organisational skills and a resilient, brave and connected mind set. The ability to travel throughout the Borough is essential and therefore a car driver would be preferable.

Search out and identify health needs and deliver skilled health promotional activities and care to individuals, families and groups in the home and community with specific health and social needs, based on professional assessment and in partnership with clients carers and other agencies.
