Job responsibilities
The Huntingdonshire Learning Disability Partnership Team have an excitingopportunity for an Adult Support Coordinator based at Scott House in Huntingdon.
The Cambridgeshire Learning Disability Partnership (LDP) bringstogether specialist health and social care services for adults with learning disabilities, enabling a multi-disciplinary approach to supporting individuals. The LDP is responsible for commissioning andproviding these services on behalf of Cambridgeshire andPeterborough NHS Foundation Trust (CPFT) and Cambridgeshire County Council.
The successful candidate will work alongside health and social care practitioners such as Social Workers, Community Nurses, Occupational Therapists, Physiotherapists, Psychologists, Psychiatrist, Speech and Language Therapists and Art / Music Therapists.
We have embraced new ways of working enabling staff to have a flexible and hybrid approach to working from home and the office. We also understand the value to being with your team in person and the support this brings.

You will be part ona team of dedicated and highly skilled professionals working on cases within community and institutional settings.As youll be working with adults, both those in need of care and support services, along with their informal carers, it will be crucial that you work in partnership with them in order to establish needs and ensure they are met.
As an Adult Support Coordinator you will complete assessments and support planning, continually looking for ways to innovate in terms of care delivery, where it benefits the service user.
This role isnt just about delivering care, its about providing service users with choice and control of their lives and empowering independence.
About youWe are looking for an energetic and motivated individual whoenjoys making a difference and building relationships. You will demonstrate excellent written and verbal communication skills, have the ability to work both independently and as part of a multi-disciplinary team. You will be committed to making a positive difference for the people we support. You must havethe ability towork from a variety of locations and travel countywide independently.
