If the answer is yes, then this is an opportunity for you.Weve changed the way that we structure our Adult Social Care teams in Cambridgeshire and have an opportunity for you to join our City and SouthAdults' Community Teamas an Adult Support Coordinatorbased at Fawcett House, Trumpington, Cambridge.

Were embracing new ways of working so our social work teams can work where they need to and have the chance to be part of a collaborative team environment with strong working relationships with health professionals and partners to achieve better outcomes for the adults we support.

The teams are responsible for supporting customers who live in the community in the designatedgeographical area, giving the ability to develop working relationships with customers, their families, and our providers. The teams works closely with our health and social care partner organisationsto promote and enable people to remain in the community for longer.

What will you be doing?

You'll need to be inquisitive and able to work with others to build working relationships with people and their support networks. You'll need to have excellent observation skills to identify strengths and work with others to promote their independence. Completing Social Care Assessments and Care and Support Planning, completing Safeguarding Investigations, Mental Capacity Act Assessments, Continuing Health Care checklists and attending Decision Support Tool meetings and engaging with Advocacy Services will be part of the role but you do not need to be an expert, full training and support will be provided.The role also involves covering the teams duty function on a rota basis and managing a diverse range of service users queries or urgent case work management.

Your curiosity should also mean that you enjoy keeping up to date with current good practice, doing relevant research and building your own knowledge.

There will be an opportunity to broaden your own skills and training by working alongside our health and social care partnership agencies,supporting providers and sharingyour knowledge and experience.
