This is your opportunity to take up a brand new, unique and exciting role working across health, social care and the wider community.

You will be:

  • leading projects to keep people well at home and maximising their independence through transforming the way we work, building relationships, reducing duplication, unnecessary admissions to hospital and delivering services closer to home
  • working in a friendly and supportive environment, alongside professionals who are committed to developing the service, such associal care teams, community health teams, statutory services & voluntary organisations, NHS colleagues, and other organisations within their designated locality
  • collaborating with partners, to design innovative and integrated community services.
