Job summary

We are seeking an experienced Specialist Community Public Health Nurse SCPHN Specialist Practitioner to play a pivotal role in delivering the Healthy Child Programme and improving health outcomes for vulnerable children, young people and families. In this position, you will undertake comprehensive assessments and lead the management of Child Protection and Child in Need cases. You will offer specialist clinical expertise, ensuring the highest standards of evidence based care while championing a child centred approach across all elements of practice.

As a SCPHN Specialist Practitioner within the 019 Public Health Nursing Service, you will provide professional supervision, guidance and support to practitioners, helping to build capability and consistency across the workforce. You will work collaboratively with multiagency partners, contribute to clinical governance, prepare high quality reports, and support the development and implementation of policies and pathways. This is an excellent opportunity for a motivated SCPHN who is passionate about reducing inequalities, influencing practice, and driving positive outcomes for the most vulnerable families.

Main duties of the job

To provide evidence based public health nursing practice that promotes and improves health and addresses inequalities of the most vulnerable and disadvantaged children, young people and families within the borough. Lead the delivery of the healthy child programme to improve health and wellbeing of children in need. Provide Specialist Community Public Health Nurse (SCPHN) expertise to develop and implement structured interventions for children and families with complex needs including assessments, appropriate management of risks and providing supervision to other team members.

Leading on and effectively managing complex situations, assessing, and managing high levels of risk ensuring step up and step down is effective in continuity of care within the service.

Adhere to and as appropriate contribute to the development and review of policies, procedures and standards relating to practice delivery, working from an evidence base or current best practice and research.

Provide clinical expertise and guidance to support decision making for vulnerable children, ensuring compliance with policy, guidance and legislation to safeguard children, including supporting staff to prepare and present complex reports, interagency referrals and adhering to safeguarding practices that promotes high standards of care.

Work within and as appropriate contribute to the clinical governance framework to maintain consistently high standards of care, working from an evidence base or current best practice.

Take responsibility for own ongoing professional learning and development, ensuring these meet local, national, and professional regulations.

Maintain accurate and up to date records safely and confidentially in accordance with policies and procedures and professional regulations.

Participate in the delivery of safeguarding training engaging with colleagues and a range of organisations to help identify and support vulnerable children and families, to promote positive health outcomes

Work within local and national safeguarding policies and procedures to ensure a child centred approach. Provide expert skills and knowledge to colleagues leading preventative work and assessments, attending meetings, preparing, and presenting reports for case conference and court proceedings as required in line with local processes.

Adhere to the professional regulations scope of professional practice and organisational policies and procedures including nurse prescribing practice and contribute and support the care quality standards and inspections.
