Job summary

Learning Disabilities & Autism Service Building Lives of Independence and Well-Being

Main duties of the job

The key responsibilities of a Social Care Practitioner are to provide person centred and outcome focused assessments designed to identify the appropriate support required for our customers that will make positive changes to their day to day lives.

You will promote customer independence, choice, and wellbeing in the community, using universal services and support where appropriate. This is a challenging yet rewarding role where you will be supported by an experienced multi-Disciplinary team.

This role is fast paced so you should be able to manage caseloads effectively and have excellent communication skills to be able to work collaboratively with multi professional teams. You will hold an NVQ Level 4 in Health and Social Care or equivalent or the ability to work towards this qualification and have experience of working within a Health or Social Care setting.
