Reablement - Empowering Independence
Become a pivotal member of our Reablement Team and have a positive impact on individuals' lives post-hospitalisation, and rebuild confidence.
Our goal is to support people to be as independent as possible. As an Occupational Therapist, you will provide professional support, train team members, and act as an expert advisor on equipment and decision-making. You will conduct strengths-based assessments and design innovative reablement plans tailored to individual needs, encouraging independence and minimising long-term care needs.
With the autonomy to manage your time and schedule, you'll be able to provide bespoke care to individuals in your diverse caseload, ensuring their unique needs are met with compassion and expertise.

Were looking for applicants with an Occupational Therapy qualification, along with a minimum of two years post-qualification experience.
Main duties of the job
The Experienced Occupational Therapist undertakes strengthsbased occupational therapy assessments, identifying customer goals and designing creative reablement plans that promote independence. They manage a busy and often complex caseload, reprioritising work to meet changing customer and service needs. The role involves completing complex moving and handling assessments and risk assessments, recording clear recommendations and demonstrating safe techniques to customers and carers. They assess for and recommend minor and major adaptations in line with the Care Act and housing legislation, using professional judgement to improve accessibility or advise on rehousing options.
The OT coordinates each customers reablement journey, maintaining regular contact with customers, families, the wider reablement team and partner agencies. They work collaboratively with health professionals, voluntary sector organisations and others to support effective intervention planning and delivery. The role includes providing professional supervision and support to Occupational Therapists, Customer Coordinators and other team members, promoting evidenceinformed practice and contributing to team development.
They are responsible for safeguarding adults, making referrals and participating in investigations as required, and ensuring all records are completed accurately and within required timescales.
