Reablement - Empowering Independence
Become a pivotal member of our Reablement Team and have a positive impact on individuals' lives post-hospitalisation and rebuilding confidence.
Our goal is to support people to be as independent as possible. As an Occupational Therapist, you will provide professional support, train team members, and act as an expert advisor on equipment and decision-making. You will conduct strengths-based assessments and design innovative reablement plans tailored to individual needs, encouraging independence and minimising long-term care needs.
With the autonomy to manage your time and schedule, you'll be able to provide bespoke care to individuals in your diverse caseload, ensuring their unique needs are met with compassion and expertise.
You'll benefit from invaluable support provided by our Principal Occupational Therapist and have access to a range of opportunities for career progression like our leadership or coaching programmes.
Were looking for applicants who hold a qualification in Occupational Therapy, along with a minimum of two years post-qualification experience.
Main duties of the job
The role undertakes complex occupational therapy assessments to identify needs and develop strengthsbased reablement plans that promote independence. It manages a varied caseload, including customers with complex physical, emotional, or social needs, and regularly reviews goals and progress.
The role completes detailed moving and handling and risk assessments, recommending appropriate equipment, techniques, and adaptations, including minor and major home modifications. It coordinates each customers reablement journey, working closely with families, carers, health professionals, and support workers to ensure safe, effective interventions.
The postholder provides professional supervision, advice, and support to Occupational Therapists and Customer Coordinators, contributing to team development and highquality practice. They ensure all work aligns with legal frameworks, safeguarding responsibilities, and council policies, maintaining accurate and timely records. The role also promotes community resources, supports carers, participates in service development, and represents the council professionally in all interactions.
