Our Learning Disabilities & Autism Service is dedicated to supporting the independence and well-being of adults and young people with learning disabilities and autism.
Main duties of the job
We are seeking a Transition Link Social Worker to support young people aged 14-18 with complex needs as they transition from children to adult services.
Within our community team and the Moving on Service, we provide social care support that empowers individuals, promoting choice and independence in their lives. Our passionate Social Workers collaborate with health professionals to deliver person-centred care that supports adults with learning disabilities to achieve their potential and maintain independence in the community.
As a Transition Link Worker, you will be the bridge in planning each young person's long-term future in collaboration with them, their families and a network of professionals.
As a qualified Social Worker, you will bring experience with health, education and social care for young people with additional needs, along with knowledge of relevant legislation like the Care Act 2014. Strong communication skills are essential, as youll build collaborative relationships with young people, carers and stakeholders across various services. This role requires a self-starter who is organised, capable of clear and accurate reporting and passionate about delivering high-quality, customer-focused support.

Operational Adult Services are structured into specialist teams in order to improve the delivery of person-centred outcomes for our customers and to evidence this through performance and set standards.
These specialist teams are:
Adult social care locality teams
Learning Disabilities & Autism Service (LDAS) / Moving on Service.
Hospital teams
Intermediate care team
Mental health team
Hearing and vision team
Emergency duty team
The community team for people with learning disabilities and the Moving on Service are social care teams who
support adults with a learning disability and people with Autism. Social Workers work closely together with
colleagues from health, harnessing a strong person-centred approach to promote choice and independence.
They work to support adults with learning disabilities to maintain their independence in the community, and to
help everyone reach their potential in life.

Essential
- Qualified Social Worker.
- Experience of and good level of understanding of health, education, and social care in relation to young
- people with additional needs.
- Up to date knowledge and understanding of relevant legislation and guidance in relation to working with,
- and the safeguarding of vulnerable adults. In particular knowledge of the Care Act 2014.
- Knowledge and understanding of Health & Social Care legislation and wider policy context.
- Well-developed interpersonal skills and ability to effectively communicate with people in a variety of ways
- and levels. Experience of working with young people and their parent / carers / adult care.
- Ability to build excellent relationships with young people, parent, carers, and professional colleagues,
- working in a collaborative manner with internal and external customers, community members and
- stakeholders that interact with this role.
- Ability to use a range of Microsoft and Electronic Social Care record software.
- A self-starter, well organised person who is passionate about delivery of high-quality customer-centric
- services.
- Ability to work with others in a developmental way.
- Must be able to write clear, complex, and accurate reports.
- Ability to professionally analyse information and provide good assessments and plans.
- Evidence of appropriate work experience and reflective learning from placement experience whilst
- qualifying as a Social Worker and of appropriate professional and personal development relevant to
- length of post-qualifying experience.
Desirable
- Evidence of post qualifying learning and development relevant to time span since qualifying.
- Completion of ASYE program.
