Are you looking for flexible hours or additional hours in your current job? We currently have the opportunity to recruit a bank health visitor to work 'as and when' within the 0-19 service in Southend. We are looking to
recruit an enthusiastic and motivated SCPHN Health Visitor to plan, deliver and
evaluate our efficient and high quality Health Visiting Service.
We work closely alongside our School Nursing
colleagues, children's centres and early years providers. We benefit from
having A Better Start within our area and our clients are able to utilise the benefits of
this service. We work closely with our partnership organisations.
The successful candidate will provide line management and leadership
within the skill mix Health Visiting Team and be proactive in delivering the
national and public health agendas in response to identified health and social
need. To work in partnership with other agencies and meet agreed standards,
objectives and statutory requirements.
Main duties of the job
You will be required to lead the delivery of the
Healthy Child Programme and ensure the 4, 5, 6, model is implemented in
practice, ensuring services are provided within the tiered model approach
Building Community Capacity, Universal, Universal Plus, and Universal
Partnership Plus.
To promote the health of all children and young
people, especially those from disadvantaged groups, to ensure they are offered
the opportunity to achieve optimum development.
To Identify, prioritise, develop and implement
programmes of care to meet individual and local needs. This will be achieved
through evidence based assessments, effective care planning and implementation
of a range of support packages for individual families, targeting the most
vulnerable families and groups utilising best available evidence.
Work in accordance to the commissioned service
specification, to ensure the 5 mandated reviews are completed within required
timescales, services provided are evidence based practice.
