We have an exciting opportunity for a Social Worker to join the Hospital Social Work Team
This is a full-time, permanent role with hybrid working, and a salary of £38,192 - £42,012 per annum plus an annual retention bonus of £3,000 subject to eligibility.
Main duties of the job
The Role:
As a Social Worker, you will be at the forefront of empowering adults to live independent and fulfilling lives. You will manage an allocated caseload, developing care plans and providing proportionate interventions in line with the Care Act. Your work will involve undertaking holistic, person-centred assessments and reviews to help residents achieve their personal goals and promoting their independence. Youll be responsible for responding to and raising safeguarding concerns, while also fulfilling key statutory duties under the Care Act, Mental Capacity Act, and Mental Health Act.

Individual needs are assessed to develop discharge plans and connect patients with necessary resources and support in the community. Assessments focuses on the whole person, not just the illness, to ensure a holistic approach to care.
