The Role:
The successful candidate will be supported by a Senior Occupational Therapist and an Assistant to manage a caseload of residents requiring assessment and intervention in preparation for hospital discharge. The role offers lots of opportunities to build on knowledge and skills around hospital pathways and support team development. Support will involve regular supervisions, competency building and annual appraisal to ensure career development.
Your role will involve:
- Provide a professional Occupational Therapy / Reablement Service, including statutory Social Care responsibilities.
- Contribute to Service delivery that is person-centred and outcome focused with an emphasis on encouraging independence and enabling recovery.
- Work collaboratively with other professionals and agencies to address complex needs and achieve best outcomes for individuals and their Carers.
- Work with health colleagues to ensure safe and timely discharges from hospital and step down beds in the community.

- Good communication and interpersonal skills, with the ability to engage with client, families, and other professionals.
- Ability to work autonomously and manage competing priorities.
- Experienced in coordinating/liaising with multiple agencies/organisations.
- Implement evidence-based practice, keeping abreast of emerging research, best practices, and national frameworks (e.g., NICE guidelines).
Desirable
- An understanding of relevant legislation, including the Care Act 2014 and Mental Capacity Act.
- Good communication and interpersonal skills, with the ability to engage with client, families, and other professionals.
- Clinical experience in assessing and managing conditions common in adult social care (e.g., frailty, dementia, neurological conditions, and physical disabilities).
- Ability to work autonomously and manage competing priorities in a fast-paced environment.
- Some understanding of specialist equipment including Assistive Technology
- Experience in working effectively with other agencies and professionals and evidence of skills and understanding of activities and work within the multi-disciplinary environment.
- Experienced in coordinating/liaising with multiple agencies/organisations in the delivery of complex cases
