These are full-time, permanent roles, with a salary of £44,496
- £48,946 per annum plus an annual retention bonus of £3,000
subject to eligibility.
Main duties of the job
The Role:
The social work team engages patients and family members in
the transition from hospital to home, with the goal of reducing adverse events
and preventable readmissions.
To execute these tasks, the team undertakes the full range
of social work tasks as required by the needs of each case and the policies of
the department in order to provide the best possible service to service users.
You will work flexibly with the management team, lead on
key projects, and chair meetings such as risk assessments and best interest
decisions. With a focus on safeguarding vulnerable adults, achieving
performance targets, and operating within budgetary constraints, you will play
a vital role in driving continuous improvement and making a meaningful
difference in the lives of our residents.

standard of service to our residents.

- oA recognised Social Work qualification and current registration with Social Work England.
- oSignificant experience in adult social care, including managing complex caseloads.
- oIn-depth knowledge of relevant legislation and policies, including the Care Act, Mental Capacity Act, and safeguarding processes.
- oStrong leadership skills with the ability to supervise and support team members and students.
- oThe confidence to represent RBWM in multi-agency settings and make sound, person-centred decisions.
- oA proactive, flexible approach to managing change and driving service improvement.
