Adult Social Care currently have a Social Worker vacancy to join our Mental Health Hospital Discharge Countywide team, on a fixed term contract for 12-months. You will work with working age adults (18-70) and older adults (70+) and will be supported by a team that cares for you, just as much as it cares for our community.

Main duties of the job
We are looking for a Social Worker to support effective and timely hospital discharges. You will work across East Sussex covering all acute hospitals for people who have been admitted to hospital to seek treatment for their mental health. This is a new enterprise, you will work as part of a remote partnership team with Sussex Partnership Foundation Trust (SPFT) and the voluntary sector together with the Adult to identify safe, creative discharges, placing the Adult at the centre.
You will be collaborating closely with other stakeholders, working together to achieve the utmost discharge plans.

1. Undertake

issues, carrying out assessments of need, analysis and planning, assessing and
managing risk, commissioning support and reviewing.
2. Identify
and appropriately report children and adult safeguarding concerns, liaising

delegated.
3. Work
with a range of legal interventions and departmental policies and procedures to
support outcome-based planning for adults.
4. Work
directly with adults in need, carers, families and their communities to support
people to make informed decisions and choices, empowering them to express needs

5. Work
with partner agencies and adults in need and their carers and families to
enable them to build networks of community support and maintain their
independence for as long as possible.
6. Maintain
good case recording and present reports to panels and in court as appropriate.
7. Contribute
to the learning of social work students, newly qualified social workers and
other team members, for example through group supervisions and mentoring.
8. Evidence
ongoing commitment to continuous professional development and undertake
relevant training in line with the Professional Capability Framework.

In
addition to the tasks outlined above, Social Workers working in Mental Health
Services will also be required to do the following:

train as an Approved Mental Health Professional (AMHP) two years after
successful completion of the formal ASYE programme.
10. To act
as an AMHP and contribute to AMHP rotas as required. This may involve
continuing to work after 5pm on duty days.
11. To
complete relevant training in order to provide practice placements for AMHP
students in training.
