Job summary
We wish to recruit two Advanced Public Health Practitioners to join our dynamic Wellbeing and Public Health team at Cornwall Council. We are looking for enthusiastic and motivated people with a passion for improving health and reducing inequalities.
This is a great opportunity to develop and influence the public health agenda across Cornwall and the Isles of Scilly whilst advancing your skills in public health.
The majority of this role can be performed through homeworking, however the successful candidate will be expected to attend face to face meetings as and when this is appropriate, these meetings will be held in Cornwall.
Main duties of the job
One post will focus on healthcare public health the development of the local Integrated Care System. The ICS in Cornwall has been involved in Wave 3 of the PHM development programme & is currently expanding to the roll out of Population Health Management across Cornwall. The role will play a key role in supporting the Consultant who leads on PHM in working with NHS colleagues & Primary Care Networks in establishing PHM methodologies in delivery of care. The role will require working across the public health team to identify & develop opportunities for closer working between public health & external partners as part of the PHM programme in Cornwall.
The other post is focussed on the wider determinants of health and their impact on all age groups. As part of a small team, you will work with other services across the Council including Planning, Transport, Housing, Economic Growth, Environment & Carbon Neutral Cornwall with a view to maximising health & wellbeing in everything that the Council does. The post will also involve collaboration with a range of external partners including the NHS, universities & the voluntary sector.
