Starting Salaries:

Level 9 £28,226 with the opportunity to progress to max £30,984 (Senior Community Care Officers)

OR

Level 10 £31,895 with the opportunity to progress to max £35,336 (Social Workers / Occupational Therapists)

OR

Level 11 £36,371 with the opportunity to progress to max £40,578 (Social Workers / Occupational Therapists)

Qualifications:Occupational Therapist or Social Worker or applicants without a professional qualification are required to be experienced Community Adult Social Care practitioners with aQCF level 4 in health and social or equivalent

With health and social care integration becoming more prevalent and the need to do things differently wed love to hear from experienced social care practitioners looking to do something a little different. This is a unique opportunity that places collaborative and direct work with people at the heart of what you do and with a focus on immediate assessment and intervention with no ongoing case management.

The Early Intervention Vehicle Service provides an urgent response to people in crisis situations within East and North Hertfordshire. The teamwork in collaboration with Urgent Care Practitioners to provide holistic assessments to help keep people safe at home, and avoid unnecessary hospital admissions. You will be based at Herts Urgent Care offices, and attend to people who are in crisis for numerous reasons including falls, infection or social care breakdown placing the person at risk of avoidable hospital admission. Given the fast-paced work you will have immediate access to assistive equipment and health care interventions to provide immediate solutions; provide information and advice and onwards referral within the rapid response team, wider Adult Care Services, health partners and wider voluntary and community organisations.

This role is well suited to assertive professionals, looking for a unique challenge, with sound crisis assessment skills and a passion for direct work with people to optimise their wellbeing in their own home. The role requires regular moving and handling and use of equipment in support of our health colleagues, so workers will need to be physically able to undertake such tasks with full training provided.

There are a variety of skill levels able to complete this role. In order to apply for the higher levels, you must be a registered Occupational Therapist or Social Worker. Applicants without a professional qualification will be considered, but they must have extensive experience of working as a community care officer.
