Job summary
With health and social care integration becoming more prevalent and the need to do things differently wed love to hear from experienced social care practitioners looking to do something a little different. This is a unique opportunity that places collaborative and direct work with people at the heart of what you do and with a focus on immediate assessment and intervention with no ongoing case management.
Main duties of the job
The Early Intervention Vehicle Service provides an urgent response to people in crisis situations within East and North Hertfordshire. The teamwork in collaboration with Urgent Care Practitioners to provide holistic assessments to help keep people safe at home, and avoid unnecessary hospital admissions. You will be based at Herts Urgent Care offices, and attend to people who are in crisis for numerous reasons including falls, infection or social care breakdown placing the person at risk of avoidable hospital admission.
