We are looking for a HCPC registered Occupational Therapist to join the one of our teams within the Adult Disability Service West based in Hemel Hempstead (Apsley). We promote personalization through our strengths-based approach focusing on the wellbeing of the individual.

Main duties of the job

You will need to be a confident individual, capable of working independently with complex cases, enabling people to live independently in the community. You will conduct holistic assessments, including moving and handling, minor and major adaptations, social care needs and will plan and care manage cases to achieve desired outcomes. You will work alongside Social Workers, Community Care Officers and Community Learning Disability Nurses to achieve the best outcomes for the individuals you work with. We are based in Apsley; however we encourage smart and hybrid working with the benefit of working from home (minimum 2 days a week in the office/home).   • North Herts/Stevenage locality teams

Provides support to adults with a learning or physical disability and we cover the East and North Herts area of the county. We promote personalisation through our new assessment process focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by offering personal budgets to enable people to direct their own support.

Older People Team East and North Hertfordshire Stevenage

  • Welwyn & Hatfield / East Herts / Broxbourne / North Herts

Our locality teams are diverse teams made up of Social Workers, Occupational Therapists and Community Care Officers. These teams focus on extended involvement of complex cases and includes general assessment and care planning as well working with our Safeguarding team to ensure risks to vulnerable people are minimised.

  • Enablement Team

Provides support to older people and we promote personalisation through our assessment process, focusing on the wellbeing of the individual.The Enablement Occupational Therapists work alongside the care providers to deliver enabling programmes. Their input is time-limited, with intervention generally up to 4 weeks. We support people in the community and on discharge pathways from Lister and Princess Alexandra Hospital.

  • Review Team

Provides support to adults with a diagnosis of dementia and older adults by reviewing those in a residential or nursing home setting. This can include systematic reviews, as well as reviews due to a change in circumstance or a safeguarding concern. The team works closely with our commissioners to complete targeted reviews when we have potential concerns with care providers.
