We are looking for a HCPC registered Occupational Therapist to join the one of our teams within the Adult Disability Service West based in Hemel Hempstead (Apsley). We promote personalization through our strengths-based approach focusing on the wellbeing of the individual.
Main duties of the job
You will need to be a confident individual, capable of working independently with complex cases, enabling people to live independently in the community. You will conduct holistic assessments, including moving and handling, minor and major adaptations, social care needs and will plan and care manage cases to achieve desired outcomes. You will work alongside Social Workers, Community Care Officers and Community Learning Disability Nurses to achieve the best outcomes for the individuals you work with. We are based in Apsley; however we encourage smart and hybrid working with the benefit of working from home (minimum 2 days a week in the office/home).
