The Adult Disability Service provides support to adults living with a learning or physical disability and our aim is to ensure we enable people to make positive decisions in their lives, promoting their independence and that they can take part in their local community. We are a service motivated by and dedicated to social care intervention and community social work.

The teams are community based and applicants will need to have a full UK driving license. We also offer hybrid working, which enables you to work from home/office (Minimum 2 days a week

Main duties of the job

  • Social Worker - Review Team West

This is a fast paced environment and the teams focus is to review care packages, develop innovative support plans, build networks and raise awareness in the area developing valuable links for the team.

Adult Disability Team East - Stevenage

The Adult Disability Service provide support to adults with a learning or physical disability and we promote personalisation through our new assessment process, focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by offering personal budgets to enable people to direct their own support. We are looking for Social Workers to work in line with Connected Lives principles in this person focussed area.

  • Social Worker - Stevenage/Welwyn & Hatfield Locality Teams:

Our locality teams comprise of Social Workers, Occupational Therapists, Community Care Officers and Nurses. These teams focus on complex cases and includes general assessments and care planning.

  • Social Worker - Review Team

  • Senior Social Worker - Welwyn & Hatfield / North Herts team / 12 month Fixed term contract or secondment in Stevenage team

If you have an interest, skills, or knowledge in working with people with Aspergers Syndrome we would love to hear from you. As an Aspergers specialist you will promote human rights through building strength-based approaches, ensuring inclusivity and removing barriers to participation and embedding personalised approaches in practice. You will enable choice and autonomy using the least intervention possible to ensure safeguarding and uphold the social work value base in practice, when working with people with Aspergers.

  • Drug and Alcohol Social Worker - Welwyn & Hatfield Team (12 month fixed term / secondment)

We are looking for a Social Worker with a specialism in drug and alcohol to work across the East of the county, to provide social care support to people with needs arising from substance use. Some experience in this field would be desirable. You will have experience of managing case work in the areas of personalisation and safeguarding. You will joint work with providers of drug and alcohol treatment and recovery services, ensuring this group of people have equitable access under the Care Act to social care assessments. You will need to be SW(E) registered Social Worker and will undertake Care Act assessments for eligible current and historical cases, using our Connected Lives strategy to review and manage packages of care.

Older People Team East and North Hertfordshire Stevenage

  • Welwyn & Hatfield / North Herts / East Herts

Our locality teams are diverse teams made up of Social Workers, Occupational Therapists and Community Care Officers. These teams focus on extended involvement of complex cases and includes general assessment and care planning as well working with our Safeguarding team to ensure risks to vulnerable people are minimised.

  • Residential review Team

Provides support to adults with a diagnosis of dementia and older adults by reviewing those in a residential or nursing home setting. This can include systematic reviews, as well as reviews due to a change in circumstance or a safeguarding concern. The team works closely with our commissioners to complete targeted reviews when we have potential concerns with care providers.

  • Community Review Team

The team work closely with the Locality teams and Post Hospital Review Teams to identify individuals who would benefit from their intervention to regain independence. The team work closely with Enablement Occupational Therapists to provide a holistic assessment.

This is a multidisciplinary team of Social Workers, Occupational Therapists and Community Care Officers, in Herts Valleys. We work with citizens and their carers in assessing their needs and undertaking care planning. We believe in putting the person at the centre of all our work and supporting them in their decisions making.

Supporting citizens over 65 years who live in a vibrant, multi-cultural town. The aim of this role to promote independence by assessing older people to identify their desired outcomes to maximise independent living whenever possible. You will undertake assessments of need to determine eligibility and implement care and support plans with clear outcomes to achieve or maintain well-being.

  • Hertsmere

Your role will primarily be to carryout comprehensive assessments to enable support and assist those in need of social care support to achieve their identified outcomes. You will promote independence and enablement for elderly people who are struggling to manage activities of daily living in their homes or in placement. Hertsmere Older person Team covers Potters Bar, Borehamwood, Elstree, Bushey, Radlett and Shenley.

We support Hertfordshire residents to return home safely. Working with their strengths, community resources and if needed a support package. We work with the multi-disciplinary team to understand the needs of our service users that we work with.   • Integrated Discharge - Post Hospital Review Team / Emergency Discharge Lister / Discharge to Assess / Lister

The successful applicants will able to evidence sound knowledge of safeguarding practice, working with people with dementia, the personalisation agenda and Individual Budgets. Able to work in a fast paced team You will keep the user and any future potential users at the centre of all activity and address user needs within the legislative framework. You will be responsible for assessing and arranging care in a hospital and community setting.
