Job summary
As aReablement Practitioner, youll be working across all disciplines within the Reablement service including sensory loss, occupational therapy and the community response team (CRT). Youll be part of a progressive team, using an enabling, adaptive approach, supporting people to live in their own homes where possible, and in the least restrictive way.
Main duties of the job
- Alongside a multi-disciplinary team of Health and Social Care professionals, youll triage referrals, complete holistic assessments of need and plan supportive interventions.
- Use a strengths-based approach when providing advice and information throughout someones reablement journey, enabling them to problem solve and make informed decisions.
- Build positive working relationships with service users and their wider support network, healthcare professionals and local community networks.
- Support professional practitioners with safeguarding cases.
About us
Working in the Reablement South Hampshire Team, the service users wesupport are primarily based within Fareham, Portsmouth, Gosport, Havant,Petersfield, Horndean, Emsworth, Hayling Island and Portchester.
Please be advised the team's current office base is at Fareham Parkway however we will be relocating to Havant Plaza in 2026.
