Job summary

Forest Courtis offering an exciting opportunity for an experienced and ambitious individual to join the team as a Reablement Lead. Here, you will be able make a real difference primarily, embedding strengths-based approaches to work with individuals to maximise their potential.

Forest Court is one of four HCC Care locations pivotal in the delivery of Adults Services strategic plan.Central to this strategy is The Discharge to Assess provision. This service supports individuals to leave hospital when safe and appropriate to do so, providing continued care and assessment outside of the hospital environment. Individuals can then receive a continued multi-agency support to achieve their desired goals and outcomes.

Main duties of the job

Forest Court is now an integral team player with hospital and community teams in providing short-term reablement and assessment placements. The successful candidate would ideally have a good understanding of this fast-paced transitional process and possess well-developed communication and interpersonal skills.

The role requires the individual to communicate effectively at all levels possessing the skills and the ability to support complex admissions and onward discharges in a professional and sensitive manner.The staffing team will look to you for leadership, and youll provide it by promoting independence and developing quality working practices and training where required.

In addition to mentoring and coaching staff, youll play a key role in building relationships with everyone from relatives and visitors to professional agencies and individuals. Underpinning everything will be your commitment to meeting the individuals needs in the delivery of a strengths based approach.

About you:

You will have a proven track record of working with older people with complex care requirements, ideally within a hospital or clinical setting. You will also be expected to hold a professional qualification such as Occupational Therapist, Physio Therapist, ROVI, RGN or Social Work Qualification.

Not least, you will need to have a positive attitude, a dynamic and fast paced worker and be a natural problem solver who is focused on creating meaningful change.

Job description

Job responsibilities

  • Monitoring the in-box and discuss referrals with the management team and rehab services.
  • Attending daily calls and liaise directly with the Hospital discharge team
  • Attending daily SPOA meetings with hospital social work teams.
  • Reviewing all pre assessments and discuss each referral with the management team, physio & OTs as required before responding back to hospital team to ensure we can meet the needs and they are suitable for a short-term service bed.
  • Coordinating with the service and hospital for each admission organise the date for admission, request medication supply, appropriate discharge paperwork and to work with the rehab team to ensure correct equipment is available.
  • Liaising with family member after MDT and continue good communication with families for the duration of the stay.
  • Chairing MDT meetings which will be attended by the therapy team and social workers.
  • Attending daily clinical meetings with clinical deputy manager.
  • Co-ordinating discharge with family, care management team and nursing team.
  • Communicating rehab goals to all staff working in the unit.
  • Communicating discharge plans with the home staff.
  • Coordinating discharges from the service update all teams and unit board.
  • Managing and update unit tracker and MDT notes.
  • Liaising with social work team to keep length of stay within the parameters of the spec wherever possible.
