Job summary

Joining our friendly and supportive Health Checks Team, you can advance your career in Public Health whilst playing a key role in preventing ill health and addressing health inequalities in our communities. We are part of the wider Hampshire and Isle of Wight Public Health Team who enjoy the challenge of working innovatively to ensure continuous improvement of public health initiatives and driving them through to completion.

Working closely with primary care across Hampshire, youll provide leadership to and support the delivery of the NHS Health Checks Programme with the aim of increasing invites and uptake, reducing inequalities and improving the quality of checks. With a passion for preventing ill health at an early stage and improving the quality of health checks, youll use your excellent relationship building and communication skills to deliver impactful MECC training and work effectively with a wide variety of partners.

This is a brand new role giving you the opportunity to put your own stamp on it, whilst working autonomously and travelling across Hampshire to work with practices. Relevant experience of working in or with general practice settings, including using primary care data systems, and knowledge of the NHS Health Checks programme is essential.

Job description

Job responsibilities

  • To work with GP practices across Hampshire to improve and standardise NHS Health checks processes, increase the quality of NHS Health Checks, reduce inequalities and support onward public health action and referrals.
  • To deliver Make Every Contact Count (MECC) training to practices, with a particular focus on staff delivering NHS Health Checks.
  • To facilitate links between practices and local healthy lifestyle services.
  • To develop and facilitate a community of learning for NHS Health Check professionals.
  • With support of the public health team, to increase the profile of NHS Health Checks within the Hampshire geography (including Hampshire and Isle of Wight Integrated Care Board and Frimley Integrated Care Board).

Operational

  • Use national and local published tools to develop a best practice framework to support Health Checks quality improvement and conversations with practices.
  • Work with practices and Primary Care Networks (PCNs) to increase the quality and reduce variability of NHS Health Checks.
  • Develop excellent relationships with local primary care providers and Integrated Care Board primary care leads.
  • Prioritise and undertake practice visits, aiming to reach all practices in Hampshire by 18 months. You will work with practices to review processes including searching, inviting, health check delivery and onward referral.
  • Work with practices to develop processes to improve links to and ensure onward referral to lifestyle services, including smoking cessation services, weight management, alcohol and substance misuse services and physical activity.
  • Work with practices to identify action practices could take to reduce inequalities, and develop best practice case studies to illustrate this.
  • To work with other members of the team develop and facilitate a community of practice learning programme for local NHS Health Check practitioners.
  • To become a MECC trainer and deliver MECC training to practice and PCN and other primary care staff.
  • To be a key public health link between practices and primary care, and you may be required to work alongside other public health programmes which have close links to primary care e.g. sexual health in primary care.
  • Review and summarise impact of visits undertaken.
  • To operate in a highly political and sensitive environment.

Information Management.

  • Utilise data to prioritise working with practices and support conversations around quality improvement.
  • Analyse, interpret and present data to highlight issues, risks and support decision making.
  • Collate qualitative and quantitative information and lead appropriate analysis to ensure robust conclusions and learning to inform training, case studies, individual practice improvement etc.

Research and Development

  • Collate a summary of evidence-based interventions and best practice to share with key stakeholders and inform quality improvement and reducing inequalities.
  • Plan, develop and evaluate methods and processes; gathering, analysing, interpreting and presenting data and information as required, including utilising data to present to practices around individual performance.
  • Develop and implement self-assessment process for practices.
  • Develop and implement evaluation of MECC effectiveness.

Resource management

This role may not manage a budget directly but work will have a significant impact of service delivery and modernisation. You will:

  • Determine short, medium and long term delivery plan, achieving quality outcomes.
  • Work across projects with programme and project colleagues to identify opportunities for coordination or further quality improvement.

Risk & Issue Management

  • Keep meticulous documentation of activity and plans.
  • Highlight exceptions and risks ensuring mitigating action can be taken to keep the programme on track. Create and oversee the project risk and issue logs.
  • Drive continuous improvement cycles to meet future needs and expectations.
  • Draft reports summarising status on issues, appraising outcomes, and providing progress reports for the Public Health Principal.

Advocacy and Champion

  • To increase visibility, awareness and support for the NHS Health Checks programme across Hampshire.
  • To support co-production and insights work where possible, working with other members of the council, Integrated Care Boards and voluntary sector to engage communities.
