Join our diverse and friendly Continuing Healthcare Team and work collaboratively with Health colleagues to support assessments for both NHS Continuing Healthcare (NHS CHC) and under the Care Act 2014, including assessments for those leaving hospital and in the community.
No two cases are the same and youll enjoy a diverse and manageable caseload, working across all client groups from younger adults, physical disabilities, mental health and older adults. Working within this complex and challenging area of health and social care law, youll have the opportunity to really stretch your social work skills and to make the most of personal development opportunities.
As a qualified Social Worker or Registered Nurse, youll ideally have experience of working in NHS CHC. If you dont have this background, youll need to have a strong desire and commitment to learn and develop your skills and knowledge, in the NHS CHC arena. Inquisitive and with strong analytical and problem-solving skills, youll really enjoy collaborating with colleagues to achieve successful outcomes within this dynamic multi-disciplinary environment.

Main duties of the job
Youll use professional knowledge, skills, and interventions in line with code of practice, statutory duties, professional practice, departmental policies and procedures and best value.
In this role youll:
- Assess the health and social care needs of adults including within the
- NHS Continuing Healthcare (CHC) assessment pathway.
- Undertake Care Act assessments and engage and support full Multi-Disciplinary Team NHS Continuing Healthcare assessments, in line with current policy and guidance relating to NHS Continuing Healthcare, NHS funded Nursing Care and local policy.
- Support the planning and arrangements, provision, or purchase, of appropriate services/care to meet a persons assessed and eligible Care Act needs.
- Work in partnership with health and social care users, carers, relatives and other agencies, to ensure a full and holistic assessment of need,including identifying current service requirements.
- Contribute to the training and development of all staff regarding policy and guidance around NHS Continuing Healthcare and Funded Nursing Care.
You will be required to travel to various locations within Hampshire (and the Isle of Wight or other local authority), to attend face to face assessments, meetings and training and it is essential that you have a current drivers licence and access to a reliable vehicle on a regular basis.We are happy to consider flexible working hours, but will need to balance work patterns with the business needs.

Assessment:
- Establish the assessed persons eligibility under both the National Framework for NHS Continuing Healthcare (CHC) and funded Nursing care (revised 2022) and the Care Act 2014 eligibility criteria.
- Work with a variety of health and social care partners to ensure a robust and holistic CHC assessment and outcome. Support individuals and their families and representatives to understand the CHC journey and remain fully involved in the process.
- Use professional skills, knowledge, and experience and a range of interpersonal and specialist communication skills to gather information and complete and record a Care Act assessment, proportional to need.
- Ensure an assessment is offered to a carer/s in relation to their caring role and completed if requested.
- Work jointly within the health and social care systems to establish and maintain robust NHS CHC assessments and implement current policy and guidance relating to the NHS CHC processes in a timely manner.
- Work in line with the Safeguarding Policies and report any concerns in line with these policies.
- Ensure that where a person lacks mental capacity to make a decision, a Best Interest Decision is made on their behalf, unless there is another person with evidenced authority to make the decision.
- Ensure that appropriate service provision is identified which is within the local health and social care systems resource framework and adheres to Best Value Principles and Choice and Equity policy.
- Joint resolution in relation to CHC decisions and outcomes.
- Offer support and guidance to individuals, carers and families and other professionals, regarding both the NHS CHC process and provisions that can be offered under the Care Act including paying for care.
