When people have had a major setback in life, its our Community Reablement Assistants at Hampshire County Council who support them to regain their independence and regain control of their life.
After a hospital admission, service users may have lost their confidence or are unable to complete daily tasks as before. Perhaps theyve had an operation or are recovering from a fall. Whatever their age, background or circumstance, our Reablement Service is designed to help service users regain the ability, independence and confidence to do some or all the things they used to do in a safe and realistic way.

Main duties of the job

The Reablement Team provide a full an in-depth supportive induction programme along with continuous training and development. There are opportunities for quick progression and fantastic prospects for career development. Some of our past Community Reablement Assistants have even gone on to becoming Occupational Therapists, Specialist Sensory Workers and Registered Managers.
A full driving licence and access to a motor vehicle is a requirement
for this role as its important to be able to travel between service users,
there is an opportunity for a car loan once you have joined us.
About us
The Reablement Service and Community Reablement Assistants work over a 7 days a week, You will be required to work evenings and/or public holidays and/or weekends as part of your Normal Working Hours. You will be required to work a shift pattern as part of your role, details of this will be confirmed by your line manager.

- To encourage service users to regaining as much independence as possible with
daily living and social_living tasks identified by the service user, Team Leaders and
Health professionals.
- To encourage and assist service users, where they are able, to graduate from
dependence to independence in activities such as personal care, skin integrity,
mobility, nutrition, supporting management of medication and communicating with
other health and social professionals.
- To provide reassurance and encouragement to service users to enable them to
manage their anxiety and regain confidence in daily living skills.
- To use appropriate techniques when working with service users who have
communication difficulties
- To be responsible for progressing service user outcome plans within guidelines set
by the Team Leader/registered professional.
- To be responsible for accurate, clear and concise documentation and verbal
communication when inputting/contributing to service users records
Corporate and statutory initiatives – equalities/health & safety/e-government/
sustainability.
- Maintains an awareness of corporate and departmental equalities, Health and
Safety policies and procedures and applies them to the day to day job
requirements.
- To maintain service user confidentiality and observe the requirements of the Data
Protection Act.
