Job summary
Joining our established, multi-disciplinary Hospital Teams youll support the development of the North Hospital Discharge Service. As a qualified Social Worker experienced in safeguarding, complex case work, resource allocation and funding, youll be passionate about working with people to achieve the best outcomes enabling people to be safe, as independent as possible and to live the lives they want.
Main duties of the job
Youll work with both health and social care colleagues to deliver excellent social work services that make a difference to adults in need of social care. Youll balance performance and drive improvement for safe, effective, and timely discharges whilst working with commissioners to maximise opportunities and utilisation of discharge services.

As an Assistant Service Manager for Adults' Health and Care, you'll provide
person centred, safe and secure services through the enactment of social care
legislation and local government requirements, informed by professional training.
You will provide operational leadership of a large social work team across several
