Job responsibilities
1. Introduction
The Least Restrictive Practice
(LRP) project, was initiated in Hampshire in 2019, and seen as one of a number
of solutions to provide high quality social care that is personalised, cost-effective,
and meets need for care of individuals with a learning disability, and or
Autism, who are eligible under the care act for social support.
The project is aimed primarily
at people with a learning disability who are in receipt of intense, complex
often restrictive care and are at risk of presenting with behaviour that may challenge.
2. Approach
It has been demonstrated that
less restrictive approaches can improve quality of life and enhance well-being of
people with learning disabilities. This
in turn reduces incidence of challenge, promotes stable living environments,
reduces the intensity of support and assists carers to maintain their
supporting role.
The programme is designed to
reduce distress/ behaviour that challenges, reducing the need
for admissions to secure hospital and restrictive levels of staff support
in health and social care through the application of the 5 key pillars of the Hampshire
Concordat;

and Minimising Restrictive Care
- Care
and Support which uses Communication strategy that meets the persons needs

of care delivery and use of Data.
The programme is designed to
develop a consistent approach across Hampshire County Council, Clinical
Commissioning Groups, Southern Health and the provider market.
This is a 12 month extension of
successful 2 year programme, and for the year 2021/2022 will now incorporate an
emerging project to focus on Trauma informed Care.
The key areas of focus for this
role are:
Working with providers to support statutory care
reviews that reflect the concordat.
Developing in partnership with care networks to
develop and review Functional Behaviour Assessments, and Positive Behaviour Support
Plans.
- Embedding positive
behavioural support approaches into working practice for social workers and
community health teams, enhancing skills, knowledge and understanding of
PBS and LRP

1. ROLE
PURPOSE
Least Restrictive Practice (LRP)
project aims to develop cross agency solutions to the provision of highly
personalised, yet cost-effective care for client with a learning disability who
present behaviour that challenges.
As a key member of a Health, Social Care, and Health Commissioning
and team, you will work under the direction of the Behaviour Specialist, to Support:
- Behaviour assessment and intervention
- Behaviour advice and
consultation to direct care teams
- Role modelling /
reflection and guidance related clients assessed behaviour and support
needs to non-qualified colleagues and to other, non-professional carers.
- Support the MDT team to implement
/ and establish fidelity of multi professional PBS / LRP intervention plans
SCOPE AND AUTHORITY
Work under the direction of Behaviour Specialist to support
their caseload management. Work collaboratively and under the direction of
other colleagues, to support the broader delivery of an interdisciplinary
intervention.
Under the direction of the team, support assessment and intervention
plans in a timely manner using best practice.
Contribute to the ongoing monitoring and progression of
intervention and identified outcomes.
Contribute to the emerging Trauma Informed Care project and
support the broader embedding of LRP principles across the health and social
care network.
2. DIMENSIONS
1. To
contribute to specialist assessments of clients referred to the team based upon
the, interpretation and integration of complex data from a variety of sources
including; historical information, Functional Assessment, self-report measures,
rating scales, direct and indirect structured observations and semi-structured
interviews with clients, family members and others involved in the clients
care, in partnership and under direct guidance of Behaviour Specialist and LRP
team members.
2. To contribute
to the formulation and of plans for the formal treatment and/or management of a
clients presenting needs, based upon a behavioural conceptualisation of the
clients situation.
3. Direct
support to care teams to implement a range of interventions based on Positive
Behaviour Support, and Least Restrictive Models of Care, feeding back to team
members to agree / adjust and refining clinical formulations.
4. To support
the evaluation of plans (Framed within an Applied Behaviour analysis, Positive
Behaviour Support, Least Restrictive Practice frame of reference)
6. To provide
behavioural advice, guidance and consultation based on agreed assessments to
other professionals and direct care givers
7. To
contribute to joined up MDT delivery of behaviour assessments and support plans.
8. To
contribute directly and indirectly to a multi-disciplinary framework of
understanding and care to the benefit of all clients of the service, across all
settings and agencies serving the client group.
9. To have an
understanding of risk assessment and risk management for individual clients and
support the assessment of need.
10. To deal
appropriately with the intense emotional atmosphere surrounding client contacts
on a daily basis and to work with frequent intense concentration for the
clinical sessions of assessment and treatment.
POLICY AND SERVICE DEVELOPMENT
- To support a data driven and
objective process of development, evaluation and monitoring of the Least
Restrictive Practice Projects intervention plans
RESEARCH AND SERVICE EVALUATION
- To utilise theory,
evidence-based literature and research to support evidence based practice
in individual work and work with other team members.
GENERAL
- To contribute to the
development and maintenance of the highest professional standards of
practice, through active participation in internal and external CPD
training and development programmes, in consultation with the post
holders professional and service manager(s).
- To contribute to the
development and articulation of best practice across the service, by
continuing to develop the skills of self and others in Positive Behaviour
Support, and Trauma informed Care, taking part in regular professional
supervision, appraisal and maintaining an active engagement with current
developments in the field of behavioural practice and related disciplines.
- To maintain the highest
standards of clinical record keeping including electronic data entry and
recording, report writing and the responsible exercise of professional
self-governance in accordance with professional codes of practice of the
Behaviour Analyst Certification Board and Trust policies and procedures.
4. To maintain
up to date knowledge of legislation, national and local policies and issues in
relation to both the specific client group and positive behaviour support.

To be based as a Team Member within the Hampshire County
Council Adults Health and Care Least Restrictive Practice Team. To provide
support and advice to Health and Social Service professionals. Line-management provided
by Clinical Team Manager and clinical supervision developed from within the
team.

Professional Leadership and Management
- To engage with colleagues
in developing a shared plan around developing least restrictive practices for
people with behaviour that challenges, or symptoms of Trauma.
- Under direction of Team,
assist in delivery of workload of behaviour specialist or other team members.
Clinical Supervision, Teaching and Training
- To receive regular professional
supervision from behaviour specialist, and other senior professional
colleagues.
- To provide support to a
range of other staff / providers / non paid carers for behavioural work as
directed by the team
- To support the team to deliver
teaching / training, as appropriate.
- Based on Team assessment provide
advice, consultation and training to staff teams.
