Job summary

To provide professional supervision and/or leadership of staff, working in partnership with the manager, staff and others and contribute towards an effective team service. To be responsible for the assessment of individual needs and the initiation and coordination of a range of outcomes to meet them, promoting the principles of a strength-based approach, personalisation, rights based social work practice, choice and control, to ensure that people can assess and manage their own needs.

Main duties of the job

1. Provide a lead practitioner role in the team: supporting the development of good, evidence-based practice particularly in the key areas of risk enablement, safeguarding, the Mental Health and Mental Capacity Act. 2. Promote a Strength Based Approach which embeds the ethos of promoting independence and wellbeing at every opportunity to ensure people can manage their own needs, risks and uncertainties to meet their short- and long-term goals, delaying the need for long term care and support. 3. Lead on professional development issues; including the encouragement of the professional development of staff through reflective supervision, mentoring and coaching, identifying and developing learning and development needs within the context of the service. 4. Provide regular and appropriate professional supervision to support, mentor and develop staff in line with approved professional standards, service and team plans. 5. Carry a reduced caseload of highly complex and specialist work, initiate and co-ordinate a range of outcomes to meet need. 6. Ensure effective completion of annual reviews, management and allocation of cases to staff. 7. Plan, manage and prioritise workload. 8. Resolve service delivery issues within available resources. 9.Prepare and present clear concise reports as necessary.

About us

You will be required to provide assessments to older adults with complex needs, who are over 65 years of age or individuals with working age dementia in the Mansfield area. We aim to help people to be well and remain living independently. Many of whom require a multi-disciplinary approach to enable them to remain independent and safely supported within the community. In addition to professional experience of working with service users and carers, you will have knowledge of the Care Act (2014), its application and its implications, which is essential to the role; and evidence of case working including Safeguarding Vulnerable Adults and Continuing Health Care is desirable, as is experience of multi-agency working to achieve positive outcomes for clients through the use of service and non-service solutions. You will be expected to undertake Mental Capacity Act assessments, Safeguarding investigations both as a safeguarding officer and safeguarding manager and complete Care and Support assessments. You will be joining our established team of person centred, passionate, enthusiastic and committed Social Work and Social Care Practitioners and Occupational Therapists. The role is based within a multidisciplinary team within Adult Social Care and Health.

An exciting opportunity has arisen in the Ageing Well Mansfield Community Team North for an Advanced Social Work Practitioner. We are looking to recruit an experienced, enthusiastic, registered Qualified Social Worker who is strongly committed to working in partnership with the people of Nottinghamshire. If you have experience and skills to supervise staff, participate in promoting a learning environment, facilitate group supervisions and provide leadership to Social Workers and Social Care Practitioners we would like to hear from you. You will have experience working with individuals with complex care and support needs, have ideally completed a qualification such as Practice Educator, Best Interest Assessor or Approved Mental Health Practitioner or be willing to work towards this. You will also be expected to have a good working knowledge, experience and understanding of the Care Act 2014, Mental Capacity Act and other relevant legislation. You will be responsible for a small but complex caseload and will be expected to undertake and complete comprehensive safeguarding investigations. You will be expected to work closely with and provide guidance to Social Care colleagues regarding adults who have care and support needs and carers providing Strengths Based, evidence based assessment and reviewing intervention. This role will also involve working in partnership with colleagues and staff from other agencies. You will be expected to be experienced in IT skills, Promote Independence, choice and control of service users to enable them to continue to live within their homes whether this is in the community or a care home environment. You will need to be experienced in completing Safeguarding Adult Enquiries, assessments and reviews in a timely manner with older people, have experience of record keeping procedures, working as a member of a team and have very good organisational skills. You will also be experienced in working to tight timescales and commissioning cost effective personalised support to meet the assessed needs of the service users as part of the reviewing process. Managing Safeguarding as part of Making Safeguarding Personal is part of the role and this can be offered as a development opportunity for the post holder. The post requires you to be able to provide structured, positive supervision to social workers and CCO, providing support in complex case work.
