Job summary
The Maximising Independence Service (MIS) focuses on early resolution, prevention and promoting independence and wellbeing for people.
Reablement is part of the wider MIS service and is targeted to meet the needs of older people and younger adults who need short term intervention due to illness, disability or vulnerability, with ordinary daily living tasks e.g. daily routines, which primarily includes support with personal care and domestic routines. It prioritises referrals from acute hospitals
Main duties of the job
1. To assess individuals' functional abilities, needs and potential for independence on discharge from hospital.
2. To design and implement reablement programmes that are person centred, have clear goals and are tailored to the individual's needs.
3. To monitor and review the implementation of individual reablement programmes.
4. To demonstrate appropriate rehabilitative/reablement techniques to people, formal and informal carers in order to maximise independence.
5. To provide and review the provision of specialist equipment and minor adaptations which maximise independence.
6. To identify where people will need ongoing longer term homecare support and indicate the type and extent of support required.

Reablement teams provide Occupational Therapy-led, individually tailored, reablement programmes to promote independence and prevent or reduce ongoing care and support needs. OTs are key to a successful, effective reablement service, using a variety of interventions and techniques to help people to regain their independence and prevent/reduce/delay the need for ongoing care.
The North START Team covers the geographical areas of Bassetlaw and Newark & Sherwood.
