Job summary

Main duties of the job

Our ambition is to

. Provide people with appropriate information, advice and support.

. Enable people to remain as independent as possible, with some short-term support if necessary.

. Incorporate peoples strengths and abilities when considering any help and support that may be needed.

. Deliver support in a way that enables people to live the life that they want while keeping safe.

. Review care and support regularly to make sure it remains appropriate.

. Treat people with dignity and respect and work with other organisations to get the best care and support possible.
