Social Care Assessor

Job summary

Devon County Council have an exciting opportunity to join the Devon Onward Care Team.

You will be based at Kingsbridge hospital as a Social Care Assessor.

• You will assist in coordinating and facilitating patient discharges from hospital back into the community for patients who have complex care needs.

• You will work aspart of a multiple disciplinary team withinKingsbridge Hospital which include, physiotherapists, occupational therapists, and nursing staff.

• You will undertake strengths-based assessments, mental capacity assessments, care management and working within the discharge to assess model.

About you

The nature of this work requires you to be highly organised with strong recording skills and in turn you will receive excellent support in a busy and friendly team.

You should be an effective communicator both verbally and through written formats, able to prioritise workload, manage conflict effectively, be flexible and enthusiastic, have good time management skills, and be able to work under pressure.

You need to be IT literate and willing to develop your skills. You will be committed to equality and diversity, to good customer service and to achieving positive outcomes for vulnerable people.
