Job summary

We are seeking to recruit an enthusiastic Care Coordinator to Join Bath and North East Somerset, Adult Social Care, First Response Team. The role is hybrid with attendance required at our offices in Peasdown St John 3 days per week.

- You will respond to initial contacts to the team, wherever possible providing information and advice to enable people to facilitate the support that they need. - You will gain an initial understanding of needs and triage to identify people who may require further involvement from adult social care. - You will coordinateand assistwith a range of services under close supervision of professionally qualified practitioners and managers.
