We are looking for a passionate Social Worker to join our Adult Social Care service, to join the AutismTeam within the Specialist Adult Social care services, which is based in Keynsham.
As a social worker you will be responsible for carrying out the responsibilities namely: to undertake assessments and reviews with vulnerable adultswith an autism diagnosis and their careers, and to work with service users to create care and support plans which meet their needs.
You will join a friendly and extremely supportive team of Social Workers, giving you the opportunity to work within a multi-disciplinary role liaising with others e.g., Psychologists, Occupational Therapists, Speech and Language Therapists and education professionals.
Main duties of the job
Your role
As a Social Worker you will manage a caseload which will include complex cases and will work independently and effectively, under the general supervision of a Team Manager.
You will carry out responsibilities including:
- Undertaking assessments and reviews of people with care and support needs and carers in accordance with the principles of the Care Act 2014
- Creating care and support plans which appropriately meet the needs of people with care and support needs and carers.
- Undertaking Mental Capacity assessments and, where appropriate, complete Best Interests Decisions in accordance with the Mental Capacity Act 2005.
- Undertaking risk assessments and work to reduce risk or engage with the person and MDT in well considered positive risk taking strategies.
